The only rule they enforce is the no bathing suit rule. Besides that you can wear whatever you want. I have gone on Carnival and Royal Caribbean cruises and have worn jeans, shorts, and flip flops to dinner, and I don't dress up for formal nights and haven't had any problems.

If you don't like the dining room dress code, you can eat the buffet on the lido deck
well not strictly but make sure its nice enough like no jeans or shorts
maybe a nice shirt not a tee shirt and maybe a skirt
YES and the main thing that is enforced is NO SHORTS during dinner...otherwise, I've seen people in jeans and t-shirts on informal nights...and no bathing suits during lunch in the dining room. Again, as stated by others, if you don't want to dress up or go by the dress code, you can eat in the Lido area or you can also order the same dining room menu from room service (for free).
