You definitely need a Passport to go to Cuba from Canada, at least. I think it is just good practice to travel anywhere with a valid passport. Just so you know, while entering and exiting Cuba they do not stamp your passport, they stamp your visitors visa, which is given to you on the plane. That way there is no evidence in your passport that you were in Cuba. At least that is what they did in Varadero.

Also atheltes participating in international/regional games, and artists (ballet dancers, musicians) traveling to Cuba to perform in a cultural exchange program are usually allowed to travel to Cuba legally with U.S. permission.
