First, Venezuela is not on the Pacific coast but the Atlantic.

If you start with Colombia on the Pacific coast then you go through Ecuador, through Peru and finally Chile all the way down to the Strait.
I will assume you are sailing from Caracas, Venezuela. The port of Maiquetia, at 30Km. from Caracas, is located in the southern Caribbean Sea and not the Pacific. You would sail north west toward Panama to cross the great canal and enter into the Pacific Ocean. From Panama you will sail south along the coast of various countries as other people indicated: Colombia, Ecuador, Peru and finally Chile. The Strait of Magellan, at the most southern tip of South America touches La Tierra del Fuego, La Patagonia, and you would also get to see Argentina whether you like or not since both Chile and Argentina share that remote corner of our world. Caracas is not a port town, you'd have to start in La Guaira, which is in fact in the Caribbean (part of the Atlantic Ocean). Travelling west (counter-clockwise) would take you first to Colombia, then to Panama, crossing the canal you'd finally wind un in the Pacific, then heading south you'd reach Colombia again, then Ecuador, Peru and Chile. The southernmost point in South America (and in any inhabited continent) is Cape Horn, a small island south of Tierra del Fuego, it belongs to Chile
