Sometimes both. When it is truely in the heart, one tends to closely follow actions that is believed to reflect worship and humility. The act of kneeling has always been a sign of respect and acknowledgement of sovereignty. Therefore when religion is in the heart and God is at the heart of religion then kneeling before God is an act of the heart.

While spirituality and religion are two different things, one is the reflection of the other. Spirituality is one's belief in a higher power and religion is the practice of that belief.

Doesn't matter how much you go to church or how pious you act, without the love of God in your heart, why even bother?
both but mostly true. There are incidents in the Bible when the people or the Patriarchs knelt on knees their to pray. It (knees prayer) is mostly done in deep prayers with hands wide open and facing heaven as when praying the Lord's Prayer.

But true religion must begin within one's heart before they can actually make it physical to convey their beliefs. Hope that helps, take care and remain blessed.
