Where do I start? What country would be the best? What is the pay like, and are there certain countries that pay more than others? What do I do about work visa, health insurance, etc?

im not reaaly familiar with everything your asking but ill tell you what i do know in latin america the best paying ones for teachers would be costa rica, panama, el salvador those are the 3 highest paying countries in central america in the order written for english teachers and you get insurance in south america brazil, venezuela, argentina, and colombia. About the work visa just go to that countries embassy and im sure they can give you 6 months to a year if not more thats all i know good luck!

A few years ago I looked for the same opportunity in Peru and most places were asking for the original language to be Spanish. I am going to Peru in month and am looking for a sharing passenger by car . I really want to drive from the USA where possible and of course ferry around Colombia. Let me know it it has some value to you to share a ride at least from Quito on to the south.
It is really hard to make a living of just teaching English in Latin America. The economy in this region is weak, salaries are bad.

You can start anywhere, if you are a native speaker, and have some credentials or experienced any English School will take you. The best country would be the most stable. You will need to fix your legal status after 3 or 6 months, and if you are working formally at any school they can help you.
There are private bilingual schools all over Latin America, most of them are Christian schools, but not all. I prefer Honduras, but I'm a little biased. I lived there for 4 years.

You won't make much, probably just enough to live on. Many of the schools will provide housing for you and will take care of the paper work for your work visa. Many also will provide one round trip ticket for you. It just varies from school to school.

One thing you shouldn't do is go into it with the expectation of making money. You will be greatly disappointed. Go for the experience of a new country, culture, and language. The only expectation should be that of the experience changing you as a person.
I am colombian and I'm pretty sure that it's so easy to get a job here. I mean the ads are full with "native english teacher needed" posts. Even I worked as an english teacher (i'm musician). The paid is not very high in dollar terms. But living here is very cheap so you can save a lot of money. If you are coming let me know (mail purollas atyahoo do tcom) and I can help you.
