I am going on vacation in jan and need to know....since I am divorced and my birth records and my drivers license have different last names do I need to take just my birth records and my license to get a past port or any other info??? thanks.

I am currently going thru the same problem. When you apply for your passport (I assume you don't have it yet)(if you do disregard this answer.) take 3 forms of identification that are older than 5 years showing the name you are currently using. Lets same my birth certificate name is Smith and my married name is Jones and all my ID says Jones, they want 3 public records showing you have used Jones for more than 5 years. I sent in my Life Insurance form (its from 1999), some DMV forms, marriage license, divorce paper (if more than 5 years ago). Anything, but not utility bills. Good Luck, they are sure making it hard. |
