Is a 2002 Spanish machine-readable passport OK to visit USA? And what if the photo now looks nothing like me?

I've a 2002 Spanish passport. Machine readable. I used to have very long hair. Now I have very short hair. So:
1. do I need a new passport or a visa to visit the USA?
2. is it a problem that my photo is now out of date?

Travel Tips
A machine readable passport from 2002 will be fine for a visit to the US. As long as you can still be clearly identified from your photo, the photo will be fine.
