Bishounen means pretty boy, or beautiful boy and on like that. Since most of the time especially in anime and manga there is a bishounen who looks very feminine and hence they are a bishounen.
Bishounen basically translates to "beautiful boy" and it's a term that is commonly used in anime and manga to describe any male character that is exceptionally, often impossibly beautiful. Bishounen (or bishies for short) are often either very tall and slender, or small and slight. They typically have hair and eye colors that you won't commonly see in the typical Japanese male (or in nature) and might be even effeminate in their beauty. Of course there are exceptions and I've seen bishies that don't fall into any of those types neatly, but it gives you a general idea.

Of course, there is the female counterpart, the bishoujo (beautiful girl) who like the bishounen is so impossibly beatiful and/or cute that there is no way a person like that could possibly exist in reality.
