It depends on where you are from. Most countries don't need a visa for a short visa but some do.
Others
It depends on your nationality. Many countries have an agreement with the Japanese government which allows temporary visas to be issued on-site. These visas (they're just stamps in your passport) allow their holders to stay in Japan for short period of time (in most cases, three months, but it depends on the country). You just go up to the customs officer and he or she will do it.

If you're American, Canadian, British, or Australian, you're covered. If you hold a passport from a different country, then check with the Japanese consulate to make sure your country is under a similar agreement. I've included a link to a site with a full listing. Enjoy Japan. It's lots of fun.

It depends on your nationality.
First of all check with your airline if it will allow you to leave the airport premises. Lot of transits will not allow passengers to leave the custom/immigration/quarantine area. And it will also depend on the time between fights, so again that needs to be checked with your ticketing airline. Then, do as everyone says, check if your country has an agreement for you to get into the actual country with or without a visa.
Just for your information, even a six hour layover may not allow you to leave the CIQ area.
First check how many hours your stopover is. If it's only a few hours, the odds are that you will not have time to leave the airport. If you are flying into Narita, the airport is quite a distance away from Tokyo and you would need to take a rapid transit train to get there, and then be back two hours before your next plane leaves. It may not be a practical option to try to leave the airport, even if you are allowed to.

Given the hightened security measures being taken nowadays, airport security is much stricter and if you have a same day flight out of Japan, the odds are that you will not be permitted to leave the airport since you would have already gone through security. If you are not allowed to leave, don't worry that you will have nothing to do. The airports in Japan have great facilities within the secured area for passengers waiting for outbound flights, with a ton of restaurants, shops, and salons. You won't have any problem keeping yourself occupied.
