The one above me was pretty close, but had a few mistakes, namely that it is a little too informal (though the level of speech is correct) and had minor translation problems. I will start from the beginning.
"Annyong Haseyo" is a question in Korean.
鞎堧厱頃橃劯鞖?
I would use it after greeting Mr. Na, instead of before.

Additionally, using 鞌?and -鞐愱矊 is a bit too informal, unless you are being sarcastic and aren't really sorry.

I would use 靹犾儩雼?and 昊?instead.
雮橃敖鞐?靹犽儔雼橁粯

There is also a slight difference between the embarrassed expressed above "彀巾敿" and the type of embarrassed you want to express (kind of adds a "how could I have done that?" to the meaning rather than a "I got caught with my pants down"), which I think is better as "鞏措Μ霊レ矤" but he will understand both.

Also, I hope you will forgive me in a more formal manner should be:
鞖╈劀頃挫＜鞁滉赴毳?氚旊瀺雼堧嫟

For your last thank you, I would add "歆勳嫟搿?quot; which is kind of like "sincerely" and commonly used in formal correspondence.
