Lost passport LS01 form info!?

I recently lost my passport and Im filling in the LS01 lost passport form.. It asks for your passport number and date of issue but I cant remember them! I know roughly when it was issued, but have no idea of the passport number! but do I leave it blank? I dont want to delay receiving a new one due to wrong information on the form! Grrrrrrr

i think you keep this blank, the passport office should be able to check these details for you, if your unsure ring the helpline to confirm

If you phone up the passport office at Liverpool and give your details then they will be able to tell you that information from their records.
