For my sister....Name on Passport.? |
Travel Info My sister is married has been for 4 years but never changed her maidan name...She still goes by her maiden name. NEVER changed it on her ss, id, driver license, anything....She needs a passport. she was born and lives in houston tx. What all information and forms does she need proof of and can she still get a passport with her maidan name or does she have to change everything...she leaves to europe in Feb 2008. italy, spain, france....maybe greece she would appreciate your help....thanks Travel Tips She will need to go to the post office with two passport photos, an official copy of her birth certificate, her driving license, and about $100. There is a form to complete, the postal clerk will review the application, and she then will mail the application packet to the address provided by the postal clerk. Go to www.state.gov and click on Travel & Business for more information. She won't have any problems applying for the passport in her maiden name since her other ID is still in that name. US passports (for adults) are valid for 10 years so she should go ahead and apply for the passport. Passport applications are taking 3 to 4 months from the day you mail the application to the day you receive the passport. She should take an official copy of her marriage license with her in case she has to prove her marital status. Others If she has never legally changed her name then she has nothing to worry about. Changing your last name is a choice when you get married but it's not necessary. If all other legal forms have her maiden name then it will be fine. |
