And the requirements above are the ones written in the forms of newly applicants (my bf is a new applicant). TWO picture I.d. at least 2 by 2. Birth certificate and nbi clearance. New application for men & single women, 3 pcs. ID picture with white background & collared shirt, NSO Secpa birth certificate, at least 2 valid IDs with signature & picture (thru travel agency). if direct to DFA, pls prepare also old documents such as voters affidavit, baptismal, school records. If married women, additional NSO Secpa marriage certificate. For renewal, old passport & 3 pcs ID pictures with white bacground & collared shirt. |
