According the first link in the source, you either get it from a Trinidad Consulate / Embassy or, if one isn't located in your country, a British embassy. The second link is to the British embassy in Cameroon.

I would suggest first trying to call one of the consulates (perhaps the Trinidad High Commission or Washington, it should be the most staffed) and find out exactly what you need. Calling the T&T Immigration office in Trinidad is extremely difficult, they answer the phone 1/20th of the time and even the first number is out of service.

If they instruct you to go British embassy as the website states, make sure to call that embassy first to make sure they can handle it.
